NICE NG185 covers the management of acute coronary syndromes including NSTEMI and unstable angina, with updated recommendations on antiplatelet therapy, anticoagulation, and revascularisation.

Key Recommendations

• Offer dual antiplatelet therapy (aspirin + ticagrelor or prasugrel) for NSTEMI unless contraindicated
• Perform coronary angiography within 72 hours for high-risk NSTEMI
• Treat very high risk features (ongoing ischaemia, haemodynamic instability) within 2 hours
• Continue DAPT for 12 months after ACS unless bleeding risk outweighs benefit
• High-intensity statin (atorvastatin 80mg) for all ACS patients
• Offer cardiac rehabilitation to all eligible patients
